SCA is sponsoring a Winter dance for our 6th, 7th and 8th grade students on Friday, December 13th, from 4:00-5:30 p.m. Students who wish to attend must be given permission by a parent or guardian by checking the box indicating compliance with the rules and expectations of your student in attendance. Payment of $5.00 must be made by 4:00 p.m. on Wednesday December 11th. Payment and Permission slips will not be accepted the day of the dance.

I understand that students must be picked up by 5:35PM. I understand that failing to pick up my student or arrange transportation ahead of time may prevent participation in future events. I give my student permission to attend Winter Middle School Dance on December 13th, from 4:00-5:30 p.m.
