List your student's name as it appears on his/her report card/Govt issued ID -- do not abbreviate or list nicknames. Notes should include student name, student number, institution name and complete mailing address or email, year of graduation. If hand carried note the date that you will pick up - must give 72 hours processing time. If you fail to provide the requested information it could delay processing time. All other request should be directed to the Annapolis HS Registrar. $2 per transcript requested
